My child was just diagnosed with a developmental disability. How do I sign up for services?

Contact Shelli Prespare-Weston or Carol Basford at 315-386-1156 in St. Lawrence County, Walt Russell at walt@cpnorthcountry.com or 518-643-0188 in Clinton/Essex County, Norm Wayte at norm@cpnorthcountry.com or 315-786-0983 in Jefferson County, and Gail Kelly at gail@cpnorthcountry.com or 518-483-0109 in Franklin County, or ask your child's physician to make a referral to our agency.  We will set up an interview with you and obtain the necessary information needed to submit an eligibly packet to Sunmount DDSO (Developmental Disabilities Service Office).  They determine if a person is eligible for the OMRDD services offered in our Family and Community Living Programs.

If your child is 0-3 years of age, he or she may be able to obtain help through Early Intervention- http://www.health.state.ny.us/community/infants_children/early_intervention/index.htm .  Contact your county Public Health Office for more information.  CP of The North Country is an approved Early Intervention Service Provider for physical therapy and service coordination in St. Lawrence County.
